We collect and use your personal information as set out below, in order to provide consultation and appointment services.
Collected through: the website consultation form, telephone and messaging channels
Once the purpose of collection and use has been fulfilled, the information is destroyed without delay and without exception. Where a statute requires retention for a set period, it is kept for that period only.
Medical records created and held for clinical purposes are retained for the separate periods set by Article 22 of the Medical Service Act and Article 15 of its Enforcement Rule.
You have the right to refuse consent, and refusing carries no penalty. If you do not consent to the collection and use of your personal information, however, we may be unable to provide consultation and appointment services.
You may decline consent for health-related (sensitive) data and still receive a general reply; in that case a specific treatment plan is discussed at the clinic rather than in advance.
Consent from a legal guardian is required for users under the age of 14. Where personal information of a child under 14 is collected, we verify that guardian consent has been given.
TONE's Clinic (the "Clinic") treats your personal information as important and complies with the Act on Promotion of Information and Communications Network Utilisation and Information Protection. Through its privacy policy the Clinic sets out how the information you provide is used and what measures are taken to protect it. Any revision to the policy is announced through a website notice or by individual notification.
